Parent/Guardian Background Information Questionnaire

Thank you for choosing Beehive Dyslexia.

This questionnaire is designed to gather background information about your child before their dyslexia assessment. The information you provide will help me understand your child's development, learning experiences, strengths and any difficulties or concerns that may be relevant to the assessment.

The questionnaire asks about areas including early development, family history, reading, spelling, writing, memory, organisation, school experience, previous support and assessments, and your child's strengths.

The questionnaire also includes information about how your data will be handled, together with the relevant consent sections. Please read these carefully before submitting the questionnaire.

Please answer the questions as fully as you can, based on your knowledge and experience of your child. If you are unsure about an answer, please provide the information you have available.
